preg_match_all returns the string being replaced, it can be used with a simple str_replace in the second instance.
makes use of update_postmeta_cache resolves gh-197
cache warming requires post data in addition to meta data.

I'm cherry-picking a couple of @peterwilsoncc's commits from #198 but leaving out the filter that addresses #190 for now.
