Add modified_date and updated_by fields to user table

Fixes #2085
Took inspiration from field doctor_experience_commenced_on on how to create a new field in the codespace Used python manage.py makemigrations and ''python manage.py migrate'' to create new field in the database table

Proposed Changes

• Add modified_date field in users table
• Add updated_by field in users table

Associated Issue

Adding these two fields will allow admin to see who made changes to user information and when!
