Add ContentUpgrade.me provider.

This adds integration with ContentUpgrade.me, which allows authors to
include downloadable lead magnets on any page.

It converts links from https://contentupgrade.me/* (e.g.
https://contentupgrade.me/kKpZyJjG) to embedded objects.
