[Feature] Move DECISIONLOG to main folder and document it's existence in README #6

Describe the Feature

Currently there is a DECISIONLOG within the models folder. However, this DECISIONLOG information is not documented in the README and can be hard to identify when reading through the use case of this data model.

We should move this file to the main folder and document in the in the README to ensure users are able to reference this document and use it to better understand how to leverage the ServiceNow data models.

Describe alternatives you've considered

Make no changes but then users are not aware that this documentation exists.
