Service update: Initiators

[joachimbrand]

The purpose of this update is to make initiator a bit easier to use and to improve the documentation.

• lomc!() now accepts a new keyword argument initiator
• InitiatorRule and concrete subtypes are exported
• concrete initiator rules have default thresholds and accept threshold as a keyword argument
• documentation improvements for initiator rules
