Actions are getting deleted from the IPOS #11

Description ->

While the user is updating his IPOS, I could see many of the actions are getting deleted as he adds new actions.

Scenario ->

User adds a number of courses and other actions in his/her IPOS.
Change it a number of times
Then the IPOS automatically deletes a few actions of the users without giving proper explanantion

Priority -> A

Expected Resolution ->

No such deletion should happen.
