Sumayya agreed to remove the Member Collection Index Page.
The app memb_collections can also be carefully deleted from the codebase.
Remove from base.settings INSTALLED_APPS and remove from urls.py.
grep the code base for references.

We can probably remove Newsletter Form page type as well.
