We have good support for sv_fps in aq2-tng ( ActionQ2 ) but when it comes to e.g. 14 players, people are saying there is "lag" when shooting. There is no such thing with fewer players on the server, e.g. with 6 players its no problem. Where could be a problem? We increased sv_max_rate to 25000

sv_max_rate just caps the client rate value. Clients either need to increase their rate manually, or minimum rate can be enforced by server for all clients via sv_min_rate cvar.
That is, assuming rate limiting is the problem (it can be verified in lagometer: rate suppressed packets are yellow).
Other than that, check server CPU and network bandwidth usage.
