Review submission should be paginated as follows (taken from mockup)

Page 2 (Atmosphere):

Do you feel supported?
Is it collaborative?
Estimate tenure rate
Estimate prof gender ratio
Do you have children? Have you felt support in this? Please comment
Have you had to deal with HR for anything? How did they handle it?
General satisfaction. Please comment

Page 3 (Compensation and Benefits):

Annual Salary
Travel allowance
