Review submission should be paginated as follows (taken from mockup)

Page 1:
These are pulled from profile if available

  • Institution name
  • Department
  • Position
  • For how long?
  • Overall experience, problems, etc
  • How do you identify?*
    Asterisk: Ethnicity disclaimer

Page 2 (Atmosphere):

  • Do you feel supported?
  • Is it collaborative?
  • Estimate tenure rate
  • Estimate prof gender ratio
  • Do you have children? Have you felt support in this? Please comment
  • Have you had to deal with HR for anything? How did they handle it?
  • General satisfaction. Please comment

Page 3 (Compensation and Benefits):

  • Annual Salary
  • Travel allowance
  • Pension (x per year)
  • Parental leave (x weeks)
  • Childcare (x amount and Onsite?)
  • Sick leave (x weeks)
  • Quality (1-10)

Page 4 (Estimate your workload)

  • Average hours/week total
  • %Teaching, admin, mentoring, grant writing, research, service, other

Page 5 (Training Program)

  • Quality (1-10)
  • Quality of teaching/training pogram
  • Your teaching load
  • Are there xx unions
  • Are you a member
    IFF STUDENT/POSTDOC
