QGIS 3.40.2 - Saving Form parameters need to click on the Apply button on each parameters field

What is the bug or the crash?

Since QGIS 3.40.2, in order to view form parameters in the attribute table, you must click on the APPLY button each time a field parameter is completed. You can't set up all the fields in one time and click the OK button, it erase each field parameters except the last one.

Steps to reproduce the issue

In the Form Attribute properties, after a set up of a field, all parameters disapears (alias, type of tool, ...) if you don't click on the APPLY button before the next field has been set.
