Add GitHub Issue and PR templates

So that I more easily review those documents

Technical Details

• GitHub offers a way to encourage users to create useful issues and PRs via Pull request and Issue templates.

Hey @JamesDCowley when you get a chance can you take a look at the PR template again? It looks like it's not showing up for me:

Created a pull request (#123) for this. Changing the name to be lowercase may work.

Last PR didn't seem to work, going to try and move the PR template file out of its own directory and put it in /.github
