Make combined query mode

Tilekiln runs a bunch of queries and concats the results. Instead, this could be done in one query. This would be an option, not a default, because it tends to result in incomprehensible error messages but it would cut down the round trips and improve performance on high latency connections (e.g. networked DB servers)
