Can you check in "C:\Users\AppData\Local\SuperPutty" and see if there is a SuperPutty.db3 file?

There is not. In fact, that directory does not exist.

It is located under C:\Program Files (x86)\SuperPutty. However, I did try uninstalling SuperPutty, removing this file, and re-installing, to no avail.

Ok, that helps shed some light on the issue. Creating the that directory and the database is the one of the first things SuperPutty does. It helps narrow down what the problem could be.
