re-publishing cancer staging info from cancerstaging.org #13

https://github.com/kumc-bmi/naaccr-tumor-data/blob/master/heron_load/csschema.sql is, as it says, largely derived from https://cancerstaging.org/cstage/schema/Pages/version0205.aspx . That page says "© Copyright 2019 American Joint Committee on Cancer. All rights reserved".

Meanwhile, much of the same material is available in https://github.com/imsweb/staging-client-csharp which bears a BSD-3-Clause license
