When running ER tool only, setting Max fps to 240 causes random enemies to die and drop bonus runes. I assume its some pathing issue. Specifically entering limgrave giving 1039 runes (the amount a farum azula hawk can drop) or 1091 when entering the blood hound dariwill dariwil fight.
Not sure why. It seems to be better with running only 200fps, and so is 239. on 239, the entering limgrave one seems to not happen, but entering dariwill does stil give the 1091. I assume there are many more spots and the game is not able to handle 240 without some enemies dying.

common triggers are dying, respawning, teleporting, entering areas, so i assume its loading in areas that causing the issue, and i dont see a way to fix it. making it basically impossible to play the game "the vanilla way" while on 240fps

Reinstalling etc, did not help.

120 fps still has the limgrave issue, I shall investigate further

Oh, it seems to be a general issue, and even happens when max fps is set to infinite as it boots up, or a base 60. so, the issue maybe lies somewhere else.

yea,, even without stutter fix or achievements disabled ticked, it still happens when entering limgrave seemingly no matter what, interesting
