Never switch cancel and action buttons

Switching cancel and action buttons makes it so user doesn't perform desired action if they aren't paying attention, but also makes user perform unwanted action if they want to cancel and instinctively to go the location 99% of the UI has the cancel button...

It's more a consistency thing, I got used to one way and when they're switched around I clicked the wrong one, though I don't remember which I preferred/is more common generally

Generally speaking the order is chosen by Qt, I'll look into these dialogs specifically though.

These are not problems with KeePassXC, you'll have to take it up with Qt or KDE:

Qt standard buttons ordering is platform and style dependent. But you may be able to override it by calling addButton() in the desired order. https://stackoverflow.com/a/37224102
Or use your own confirmation box. Or follow the platform defaults in the other dialogs.

Don't know if any of it is worth the effort, though.
