Nbgrader feedback files contain a table of contents with links to each task in the file. Clicking such a TOC link shows a blank page due to a frame-ancestors issue. See JupyterHub security explanation page for some background. Maybe related to jupyter/nbgrader#1830.

No idea how to resolve this.

The formgrader lab extension uses iframes, which is not supported by JupyterHub. There are some workarounds, but not real solution. For the time being, open the formgrader in a new browser tab. Then everything works as expected. See Nbgrader doc.
