player was killed by shulker. their head was in their grave, but their netherite helmet was missing

Thanks for reporting an issue.

This sounds like an issue with the graves plug-in. HeadDrops does not alter the player’s death drops; it only appends their head item to it. I’d recommend reporting this issue to the developer of the Graves plug-in.
