Ability to deselect a program while creating an encounterType

To Reproduce

Steps to reproduce the behavior:

1. Click on "New encounter type" to create a new encounter type.
2. When selecting a program, the operator/user is unable to deselect the selected program.(This happened with a self-service user who had selected a program by mistake)

• When there are no program encounters of encounter type that is under Edit, clicking on the cross icon(like we are having in all search text boxes) should unselect the selection.
• or shall we do another selection of same option to be able to unselect it - like present for User groups in User page

Inputs:

• cross is more visible
• disallow - when there is data. -> later - assign/unassign
• subject type - should also not be editable
• bundle upload
• form mapping

Inputs based on testing

• During de-select operation done for Program, EncounterType, ensure that its a proper void of form mapping
• During change from one Program / SubjectType / EncounterType to another corresponding Program / SubjectType / EncounterType, we should void the previous form mapping before creating a new one
