Add Rails 4 information #30

Rails 4 has been out for quite some time and hopefully people start to pick up rails 4. There are a couple of things to address here:

new directory structure (e.g. the new concerns directory plus other changes etc.)
maybe strong parameters vs. attr_accessible?
the migrations pending error?
