condition_variable timed_wait failed / acquireRenewalLock timed out

Reported from the C++ MSL stack. When these timeouts occur, the stack may crash.

Leading suspicion is that when the renewing request takes longer to finish than typical (e.g. network issues) and a second request is blocked waiting for renewal to complete or to acquire the renewal lock in order to perform renewal then something bad is happening.
