Error with chordal axis

When running the chordal axis plug in, I receive a Python error:

UnboundLocalError: local variable 'centre_lines' referenced before assignment

Is this a problem with the script included in the plug-in or is this something that relates to my configuration? The full message is below:

I am receiving the same error. It is also producing the same result as mentioned in other previous issues; having a significant portion of the polygons end up with no axis drawn, but they are being tessellated. I've attempted some of the other suggestions with no success.
