Reproduction:

One of the mutations I researched has all of the GCATs in the correct order, and no orange outlines, and the mutation info panel is blank, even when opening and closing the console. (Along with the DNA icon showing as unknown still)

Possible cause is that the unknown mutation was incompatible with one the target already head,. Like giantism and dwarfism. IIRC this makes it so you can't discover it untill you remove the incompatible one

Can confirm it, gave myself gigantism via VV and scrambled till I got dwarfism.

When you "Solve" the incompatible gene it'll remove all the orange outlines but not reveal untill you remove the other gene. And the target gets a message saying "You feel your genes resisting something"
