upgrade CAMFIVE manual for printing

Prior manual was corrupt and would not print; PDF/A renditions had failed characters.
New replacement prints. Updated as follows:
- created replacement by capturing images and pasting into Apple .Pages document
- adjusted page numbering placement for double-sided printing
- adjusted to running header
- adjusted other design elements for reduced ink usage
