I've noticed problems blocking auto-play on sites that use a pop-up HTML5 video player, like used on media outlets like TheHill.com, CNN, WashingtonPost, etc. The autoplay seems blocked when the video is embedded into the page, but these websites have started popping open a new window within the page if one scrolls past the original video player.
Here's an example. My settings say this should be blocked, but if I scroll down beyond the video on the page, the embedded new window will play automatically.
