When generating constraints from normalising expressions, duplicates can arise. This can also arise for generic expansions. Here's a canonical example:

(column X)
(column Y)

(vanish tmp1 (~ X))
(vanish tmp2 (- 1 (~ X)))
(lookup test (Y) ((~ X)))
