Files not uploading and not downloadable #24

Hi - documents are not uploading to the contact

see attached shows the subject, but the file is not downloadable
the file can't be found in files/civicrm/custom

In the database

there appears to be a record in civicrm_documents
but not in civicrm_file

Specs: Civi 5.26.2, Drupal 7.72
Ubuntu 18.04, PHP 7.2.24-0ubuntu0.18.04.6
